There is a low-cost or no-cost fitness pass option for insurance-reimbursed fitness programs, including:
New, starting January 1, 2026:
- One Pass, as part of AARP Renew Active Optum Plan, including Moda Health, PacificSource and Providence Health Plans
Existing Programs
- One Pass AARP Renew Active
- One Pass Optum Renew Active
- Active & Fit Enterprise

- Silver & Fit
Eligible programs will include unlimited access to BPRD fitness centers, virtual fitness classes, lap swimming and drop-in fitness classes, including water fitness classes and workout-on-your-own sessions.
How to renew Medicare/employee health benefit-reimbursed passes:
The Medicare Open Enrollment Period also known as the Annual Election Period (AEP) for Medicare health and prescription drug plans happens only once per year.
Medicare Advantage plans offer additional benefits not covered by Original Medicare. Plan options may include coverage for prescription drugs, dental, vision, hearing, fitness and more.
During Medicare open enrollment, sometimes called the Medicare annual enrollment, you can change Original Medicare, Medicare Advantage or Medicare Part D plans, including changing your coverage, switching between plan types and adding prescription drug benefits.
Enrollment for Medicare enrollees who want to make changes to their coverage happens every fall between October 15 and December 7. Any changes you make during the 2025 Medicare enrollment period will affect your 2026 Medicare plan, beginning January 1, 2026.
Any new pass holders, or any returning patrons with Moda Health, must fill out and return the enrollment form. You will need to confirm your eligibility with your provider and ask for an eligibility/fitness number to complete enrollment. Complete and return the PDF, or fill out the online form below.
If you’ve changed your insurance or personal information, you must fill out and return the form. This includes:
- Patrons that have Moda Health, as Moda Health plans have changed to One Pass for 2026.
- Patrons that changed supplemental insurance companies.
- Patrons that changed their address, phone number and/or email address.
If you are eligible and once you have your eligibility/fitness number, complete your enrollment form by either:
- Completing an electronic form at: bendparksandrec.org/medicareform.
- Print the PDF enrollment form and either mail or hand deliver the completed form to customer service at:
- Larkspur Community Center, 1600 SE Reed Market Rd.
- Juniper Swim & Fitness Center, 800 NE 6th St.
- BPRD District Office, 799 SW Columbia St.
If the form is filled out with all accurate and all pertinent information, your facility pass will be activated within 3 – 4 business days and will be notified via email.
Frequently Asked Questions
Q: I already have a Medicare/employee health insurance benefit-reimbursed fitness pass for 2025. Do I need to renew?
A: If you have the same insurance and personal information, it is likely your pass will continue to be active. Answer the following questions to know for sure:
- Is your insurance the same?
- Do you not have Moda Health?
- Is your phone, email and address the same?
If yes to all of the above, you do not need to renew for 2026. Your 2025 pass will automatically continue into 2026.
If no to any question, you will need to renew with BPRD by completing an enrollment form. Enrollment forms will be processed after Jan. 1. 2026, Please allow 3 – 4 business days for processing and you will be notified via email when finished.
About Moda Health: Moda Health is changing from Silver & Fit to Optum One Pass with Medicare for 2026. Due to this change, you will need to renew with BPRD by completing an enrollment form. Be sure to get your new fitness benefit number – it starts with an “A” and is followed by nine digits. Enrollment forms will be processed after Jan. 1, 2026. Please allow 3 – 4 business days for processing and you will be notified via email when finished.

Q: I changed my insurance provider. Do I need to renew for 2026?
A: Yes. If your insurance provider has changed, you will need to renew with BPRD by completing an enrollment form. Be sure to get your fitness eligibility number from your new provider. Enrollment forms will be processed after Jan. 1, 2026. Please allow 3 – 4 business days and you will be notified via email.
Q: My insurance is with Providence Health Plan. Do I need to renew for 2026?
A: Yes. You will need to renew. Providence Health Plan is changing from Silver & Fit to Optum One Pass with Medicare for 2024. Due to this change, you will need to renew with BPRD by completing an enrollment form. Be sure to get your new fitness benefit number – it starts with an “A” and is followed by nine digits.
