HEALTH INSURANCE:
The district offers a PPO insurance plan through PacificSource. To find a preferred provider through PacificSource, please go to www.Pacificsource.com. Once you are on the website, go to the Provider Directory and follow the instructions to select a preferred provider.
Full-Time Employees: Are eligible to enroll with no more than a one-month waiting period. The district pays 90-percent of the monthly premium for full-time employees and 75-percent of the premium for dependents.
Part-Time Employees: May enroll based upon position eligibility.
DENTAL INSURANCE:
Full-Time employees: The district is self-insured and offers a traditional dental plan managed by QVI Risk Solutions, Inc. The district pays 100-percent of the monthly premiums for full-time employees.
Part-Time employees: May enroll based upon position eligibility.
VISION INSURANCE:
Full-Time Employees: The district is self-insured through QVI Risk Solutions, Inc. The district pays 100-percent of the monthly premium for full-time employees.
Part-Time Employees: May enroll based upon position eligibility.
VOLUNTARY LIFE INSURANCE
Full-Time Employees: The district provides a $50,000 Term Life insurance policy through UNUM Assurance Company.
LONG TERM DISABILITY (LTD)
The district provides a long-term disability plan, through UNUM Assurance Company for full-time employees.
SUPPLEMENTAL INSURANCE
Through Colonial Life, employees may elect to purchase Short Term Disability, Cancer Protection, Accident Benefits, Intensive Care, Life Insurance, and Long-Term Care.
EMPLOYEE ASSISTANCE PLAN
The district provides a confidential employee assistance program to all employees. Premiums are 100-percent paid by the district and coverage includes three counseling sessions per year for the employee and eligible dependents. Services are provided by St. Charles Behavioral Health and are confidential to the fullest extent provided by law.
FLEXIBLE SPENDING ACCOUNT
Enrollment in the Flexible Spending Account (FSA) plan gives employees the opportunity to pay for certain expenses on a pre-tax basis, such as dependent care costs and out-of-pocket medical and dental expenses.
DEFERRED COMPENSATION
Employees may voluntarily participate in deferred compensation through Nationwide and/or AIG Financial Services. Deferred compensation plans are 457 plans that allow employees to defer a portion of their salary on a pre-tax or post-tax basis until retirement.
PUBLIC EMPLOYEE RETIREMENT SYSTEM (PERS)
Eligibility: After completion of six full calendar months, with a minimum of 600 hours in a calendar year, employees will automatically become a member of PERS.
Contributions: The district pays the employer contribution (based on salary) to this retirement program for employees. Employees contribute 6-percent of their pay into the Individual Account Plan.

ALL ACCESS FACILITY PASS: MEMBERSHIP TO JUNIPER SWIM AND FITNESS CENTER
Employees receive a pass to use district facilities including The Pavilion, Juniper Swim & Fitness Center and the Senior Center. Full-time employees receive a family membership upon hire and part-time employees receive a family membership upon completion of six months.
BEND PARK AND RECREATION DISTRICT PROGRAMS
Employees working at least 50 hours per month for six months receive a discounted rate for eligible programs.
VACATION LEAVE
Full-Time Employees: Accrue paid vacation leave at the completion of one full payroll period. Accrual rates increase with longevity with the district.
Part-Time Employees: Eligible employees will accrue vacation leave on a pro-rata basis.
SICK LEAVE
Full-Time Employees: Accrue 8 hours of sick leave per pay period.
Part-Time Employees: Accrue paid sick leave at a rate of at least 1 hour per 30 hours worked.
HOLIDAYS
Full-Time employees and eligible part-time benefited employees receive the following holidays:
- New Year’s Day – January 1
- Martin Luther King, Jr. Holiday – Third Monday in January
- President’s Day – Third Monday in February
- Memorial Day – Last Monday in May
- Independence Day – July 4
- Labor Day – First Monday in September
- Veteran’s Day – November 11
- Thanksgiving Day – Fourth Thursday in November
- Friday after Thanksgiving
- Christmas Day – December 25
PERSONAL LEAVE
Full-Time employees receive one personal leave day each calendar year upon completion of their orientation period.
Part-Time eligibility will depend on position.
DIRECT PAYROLL DEPOSIT
Employees are encouraged to have their paycheck deposited directly into their bank or credit union. Forms are available from Payroll or Human Resources.
OTHER BENEFITS
Employees may participate in the Mid-Oregon Federal Credit Union.
